Dr. Loftin's office is located off exit 74 on I-65
near Brentwood.  From I-65, take exit 74A, travel
approximately 0.6 miles on Old Hickory Boulevard
from the interstate.  The office is on the left at
750 Old Hickory Blvd in a two-story gray building
labeled "TWO."  You may turn left at either the first
or the second red light after the Target shopping
center.  There is a large parking lot beside the two
buildings in the office complex.  The office is on the
first floor in Suite 150.
